Senior Solutions is more than just a set of services—it’s a holistic approach to successful aging that focuses on understanding and addressing the specific needs of the individual. We start by sitting down with you and your family to assess your current situation and identify areas where support is needed. From there, we create a personalized plan that outlines the steps needed to achieve greater independence and well-being.

Our Process:

Assessment and Planning Tailored to You. Each individual has different needs and goals; our first step is understanding them. Our care managers assess your situation through in-depth consultations, considering everything from health and mobility to social engagement and financial stability. We develop a detailed plan that aligns with your goals and addresses your needs to remain safe and independent.

Coordinated Care: Bringing Resources Together. Once your customized plan is developed, you may choose to engage our ongoing support to implement it. We actively coordinate the services and resources you need, drawing from CJFS programs and trusted external providers. Whether arranging for in-home care, collaborating with healthcare providers, or communicating with family, we ensure that all aspects of your care are seamlessly integrated.

Our Core Services:

• Holistic Care Management. Our care managers provide ongoing support, helping you navigate challenges and connect with necessary services. From accompanying you to medical appointments to assisting with personal affairs, CJFS makes certain every detail is addressed.
  
• Need-Based Personal Care Support for Daily Living. For those who need extra help at home and meet income eligibility limits, our personal care attendants provide affordable assistance with tasks like meal preparation, bathing, laundry, and light housekeeping. These services are designed to support your independence, ensuring you can continue living comfortably in your own home.
  
• Safe, Reliable Transportation. We offer two transportation support options. For adults over 60 and/or adults with disabilities, our Buz-A-Bus service provides door-to-door transportation within a 5-mile radius of our CJFS office. Our Escorted Transportation service is specifically designed to transport and accompany you to medical appointments.
  
• Home Safety Assessments. Our certified Senior Home Safety Specialists (SHSS) provide in-home assessments to identify potential hazards. We offer recommendations to help you navigate your environment more securely, such as installing handrails in the bathroom or making sure your house has adequate lighting.
  
• Financial Support Services. Managing finances as an older adult can be challenging, but CJFS is here to help. We provide budgeting assistance, coaching with bill management and coordination with accountants.
